Summary
Older adults with Alzheimer’s disease and other types of dementia are more likely to be hospitalized. But when health care providers help people with dementia manage their condition, it’s often possible to prevent or shorten these hospitalizations. Evidence suggests that coordinated care models in primary care settings can reduce the risk of hospitalization in people with dementia.
