Healthy People State and Territorial Coordinators

The Healthy People State and Territorial Coordinators make Healthy People happen every single day across the United States. Each state or territory has a Healthy People Coordinator who serves as a liaison with the Office of Disease Prevention and Health Promotion (ODPHP). ODPHP works with Coordinators to identify areas of alignment in their work and the Healthy People 2030 goals and objectives. They also collaborate with the HHS Office of the Assistant Secretary for Health (OASH) Regional Offices. State and Territorial Health Improvement Plans

Healthy People has linked to the most recent available version of each state’s and territory’s improvement plan. Explore your state’s or territory’s improvement plan below to learn how it addresses the Healthy People 2030 Leading Health Indicators (LHIs) affecting your community. Plan implementation may not align with the Healthy People 10-year cycle and may still be in progress, evolve over the decade, or be complete. Please check back regularly for updates on available health improvement plan information.
